The theory has been widely adopted in trauma therapy, where its language of nervous system states has provided practitioners with useful clinical scaffolding. The underlying neurophysiology, particularly the claim about a phylogenetically distinct mammalian ventral vagal pathway and its role in social engagement, has been seriously challenged by mainstream physiologists who argue Porges' specific anatomical and evolutionary claims do not match the evidence.
