Classical model of infectious disease causation.

The Epidemiologic Triad (Host-Agent-Environment) is the classical framework in infectious disease epidemiology articulating that disease occurs through interaction of three elements: (1) Agent — the infectious microorganism (bacterium, virus, fungus, parasite) or other harmful exposure causing disease; (2) Host — the human or animal in which disease occurs, with characteristics affecting susceptibility (age, immune status, genetics, behavior, comorbidities); (3) Environment — the external conditions affecting agent-host interaction (climate, sanitation, vectors, social conditions, healthcare access). The framework substantially extends germ theory (separately enriched, Pasteur and Koch's foundational work establishing microbial causation) by recognizing that disease causation is multifactorial — the same agent doesn't produce disease equally in all hosts or environments, and host susceptibility and environmental conditions substantially affect whether and how disease occurs. The triad emerged from substantial 20th-century epidemiology, with foundational systematization through Wade Hampton Frost (early 20th century), John Gordon (mid-20th century epidemiology textbook tradition), and broader epidemiologic education. The framework is sometimes extended to four elements adding 'time' as separate dimension (epidemiologic tetrad) or to broader 'web of causation' frameworks acknowledging substantially more causal factors than the classical three. The triad is foundational to infectious disease control through interventions targeting each element: agent-focused interventions (antibiotics, antivirals, vaccines targeting agent-specific immunity); host-focused interventions (vaccination boosting host immunity, behavioral protection, immunocompetence support); environment-focused interventions (sanitation, vector control, contact reduction). The framework remains foundational in epidemiology education and infectious-disease practice, though contemporary infectious-disease epidemiology substantially extends it through molecular epidemiology, network analysis, and broader social-determinants integration.
